Welcome to Five Valleys Urology. Our 4 physicians, along with two physician assistants and our highly qualified staff, are pleased to provide patient care and surgical services to patients from throughout the region. Our medical doctors have privileges at Saint Patrick’s Hospital and Health Sciences Center, Community Medical Center, St. Luke’s Community Hospital (Ronan), St. Joseph Medical Center (Polson), and Marcus Daly Memorial Hospital (Hamilton). Our office is conveniently located on Missoula’s northwest side, just one block west of North Reserve Street behind Famous Dave’s restaurant.
To schedule an appointment, you may call or email your request (email@example.com). Our office staff will schedule your appointment promptly. We provide same day care for emergent patients and most patients can be seen the same week they call. In order to schedule your appointment we’ll need to gather personal information about you such as your date of birth, social security number, address, health insurance ID number, etc. We take your privacy seriously and have identity security measures in place to protect your personal health information. Many appointments are scheduled as referrals from your primary care physician and we’re happy to coordinate care with your regular doctor. We’ll provide your doctor with feedback on your visit, normally within 24 hours.
Your First Visit
In order for our physicians to provide you with the best care, it is important we gather information about your medical history, medications, and your current urologic problem. In addition, we bill insurance companies as a courtesy to our patients so current and complete health insurance information is necessary.
All new patients are required to complete our patient registration forms and either send them to us before their visit or bring them in when they come. (Forms are available on our DOWNLOADABLE FORMS PAGE.) Completed forms can be returned by email, fax, or postal service. You may also bring the completed packet with you to your appointment. Please note: If you chose to mail your packet please allow three days or more for delivery. If you mail your forms, keep in mind you may have to fill them out again if we haven’t received them by the time of your appointment.
Once you establish care with us, we will update your information periodically to make sure our records are current. Our nursing staff will update your medications and conditions with you at each visit before you see your provider. Finally, to comply with federal rules designed to protect your identity, we ask patients to bring their health insurance card (if insured) and a photo ID to each visit.
Insurance & Billing
Our doctors are participating providers for Medicare and Medicaid as well as many commercial insurance carriers. Because health insurance is a contract between you and a health insurance company, we advise our patients to check with their insurance company or employer to determine coverage or limitations prior to appointments. It is also very important that we have the correct insurance information or we won’t be able to bill the insurance company. Patients who do not wish to provide us with all the information we need to bill insurance will be treated as self pay patients. As a convenience to you, we bill your insurance company for services rendered. Please remember, new identity theft protection laws require us to ask to see photo identification and health insurance cards at each visit and insurance companies require us to collect co-payments at time of service.
For patients with health insurance, we collect co-payments at time of service and will submit an electronic claim to your insurance company. Patients receive detailed monthly statements and are required to make patient responsibility payments within 30 days of the receipt of their statement. Patients who are uninsured or self-pay are required to make a $100.00 down payment at time of service and any remaining balance will be billed at the time of statement. We do offer prompt pay discounts for self-pay patients.
